Has anyone got a copy of the original original Jan 1998 PS1 release of Resident Evil 2 that has the Sweepstakes promo on the cover AND has the original entry card still inside?

If so could they please take a photo or scan the entry card and either reply or PM me the links?

I'm working on something right now that I forgot to look up the info for the sweepstake thing on. It'll make sense once I post it.
