There's Wesker's Report 2, which was in the Japanese version of REmake, I believe.

Wesker's Report I included with pre-orders of Code Veronica X for the PS2 given out by Electronics Boutique and Game.uk.com in UK.
Wesker's Report II -A website with both JAP & English versions where available.

Biohazard Collector's Box - I think this is just a typed up version of Wesker's Report I.
I think either the JAP version of Wesker's Report I or the one re-released on the anniversary and boxset has the line about Sherry removed.

I already ripped the flash version when it was online I extracted the original .FLV, .SWF or something I can't remember but I know I ended up with all the .JPG's ripped directly from the site, no PrintScrn screenshots..

I remember the black case with the coffee-cup like stain on the front being on eBay really cheaply. I never got it because I thought it was a bootleg. Goddamn.
